ภาวะภูมิคุ้มกันบกพร่อง หรือที่เรียกกันทั่วไปว่าระบบภูมิคุ้มกันอ่อนแอ คือภาวะที่ความสามารถของร่างกายในการต่อสู้กับการติดเชื้อ อาจเกิดจากปัจจัยหลายประการ รวมถึงพันธุกรรม การติดเชื้อ การเลือกวิถีชีวิต และปัจจัยด้านสิ่งแวดล้อม ภาวะภูมิคุ้มกันบกพร่องอาจเกิดขึ้นได้ตั้งแต่แรกเกิด (แต่กำเนิด) หรือเกิดขึ้นภายหลังจากการดำเนินชีวิต
ภาวะภูมิคุ้มกันบกพร่อง ร่างกายจะไม่สามารถหรือมีความสามารถไม่พอต่อสู้กับเชื้อโรคหรือสิ่งแปลกปลอมอื่นๆ ที่เข้าสู่ร่างกายได้ อาจทำให้เกิดการติดเชื้อบ่อยครั้งและรุนแรงซึ่งรักษาได้ยากและอาจต้องได้รับการรักษาด้วยยาปฏิชีวนะในระยะยาว นอกจากนี้ยังอาจทำให้ร่างกายหายจากบาดแผลหรือการบาดเจ็บได้ยากอีกด้วย ในบางกรณี ภาวะภูมิคุ้มกันบกพร่องอาจนำไปสู่โรคแพ้ภูมิตัวเอง โดยที่ร่างกายโจมตีเนื้อเยื่อที่มีสุขภาพดีของตัวเองโดยไม่ได้ตั้งใจ
